## Narrative

Employee was working on the bed of a bucket truck approximately 5-ft high. He was tightening a bolt on the truck with a wrench; the wrench slipped, causing him to lose his balance. He fell off the truck and hit his head.  He was taken to St. Francis Hospital.  He was in the emergency room and then transferred to the intensive care unit.
