## Narrative

While attempting to retrieve a piece of plastic from a conveyor, a machine that was dumping  a mushroom bin onto the conveyor hit the employee. The employee got caught between the guardrails and the dumper, puncturing the employee's lungs and breaking an arm and two ribs.
