## Narrative

An employee was using a pneumatic jack to raise up a tractor truck. Once it was raised, and while he was attempting to place the jack stand under the leaf spring, the truck slipped off the jack.  The truck trapped the employee's right hand between the leaf spring and the jack stand, crushing and amputating his right index finger.
