## Narrative

An employee was removing an old pole to replace it with a new one. The chain that was holding the old pole was out of center. The employee grabbed the pole and he was lifted. Then he fell from the pole and landed on the outrigger of the truck. The employee was hospitalized for chipped vertebrae in his back.
