## Narrative

An employee was working from a 6-foot step ladder while using a screw gun to install a panel cover. He lost his footing, knocked the ladder from beneath himself, and fell to the ground, landing on top of the ladder. He suffered a head laceration and wrist injury, requiring hospitalization.
