## Narrative

An employee was cleaning a food cart in the back storage part of the store. A forklift operator on the other side of the storage rack was pushing a skid with a cooler but pushed it too far, causing a pallet (weighing approximately 100 pounds) on the other side of the rack to fall on the employee cleaning her cart.
