## Narrative

Employees were setting up a piece of concrete block manufacturing equipment using a telehandler to raise a section of fabricated steel. A piece of shipping pallet that was still attached to the steel fell from overhead and struck an employee in the head knocking him to the ground. The employee was admitted to the hospital for a concussion.
