## Narrative

While servicing a valve, an employee sent a solenoid to the valve to open it and see if the rubber seat was damaged. The solenoid coil disconnected, which interrupted the solenoid signal and caused the valve to close on the employee's thumb. The thumb had to be surgically amputated.
