## Narrative

On Friday 10/21/2016, at 0900, an employee was on the second story of a building replacing a seal on a window. The employee went to take another step up when the ladder gave out beneath him and he fell to the ground, fracturing his right and left femurs and injuring his ankle. He was hospitalized.
