## Narrative

A pallet was jammed at a pallet magazine discharge area. An employee attempted to unjam the pallet using a hook attached to a rod about 36 inches long. When the hook did not work, the employee grabbed the pallet with her hands. At the same time, the machine cycled the next pallet in place, causing the pallet that she was grabbing to raise. Her left thumb became caught between the pallet and the metal frame of the machine, amputating her thumb tip.
