## Narrative

An employee was attempting to change the buffing blades on an automatic tire buffing machine. While he was using a wrench to remove the blade head, the machine turned on. The wrench became entangled in his clothing and pulled him into the machine, breaking his right wrist and lacerating his right forearm, bicep, and leg. The machine was not locked out at the time of the incident.
