## Narrative

On 11/30/16, an employee was pulling pipe from a well when the pipe was released from the elevator before the arm that held it was in place. The pipe fell and struck the employee's right hand, crushing his ring finger above the knuckle and fracturing his middle finger.  His ring finger had to be amputated.
