## Narrative

An employee was investigating the discharge of a rotary airlock.  He removed the inspection door, visually inspected the spout, and thought he saw a suspicious buildup.  When he attempted to remove the buildup inside the spout with his left hand, his glove was caught by the running airlock, which then pulled his hand in and amputated three fingertips.
