## Narrative

On 10/26/2017, an employee was driving a walk/ride truck to load ice cream onto a trailer. The employee was traveling north toward a freezer doorway while a second employee operating a standup forklift was traveling westbound to the freezer. The trucks collided at an intersection, causing the first employee to fall to the floor.
