## Narrative

An employee was working on an overhead door while standing on a 10-foot rolling stair unit with a wheel locking mechanism.  A reversing forklift ran into the rolling stairs, which were knocked over, and the employee fell 10 feet to the ground. He broke his right elbow and his right hip, requiring hospitalization and surgery.
