## Narrative

While repairing a brown stock washer, an employee was attempting to remove a tie rod from the packing with a come-along when the tie rod fell. The employee reached out to grab the packing and his right fourth finger was crushed between the tie rod and a hydraulic shaft, resulting in a finger amputation.
