## Narrative

An employee was performing maintenance on a wireline tool that was hooked up to a control panel. He closed a panel on the tool with a command at the control panel. As the panel was closing, he was turning the tool and his fingers were caught in the backup shoe of the tool resulting in an amputation injury.
