## Narrative

An employee was in the back of his trailer offloading a 1500 lb. crate for a customer. There were two straps on the crate holding it to the wall. He released a strap so the customer could unload the crate completely. When he released the strap, the crate tipped over onto him. He sustained a broken shoulder, broken ribs, and an ear contusion.
