## Narrative

An employee was installing a lower lot hatch cover in the lock out trunk area inside a submarine. The employee attempted to insert the top bolt through the yoke and hatch. When the bolt was not aligning correctly, the employee tried to adjust it. The bolt moved and the employee's left index finger traveled upward as the alignment of the hatch shifted. The hatch contacted the employee's finger, resulting in amputation at the first knuckle.
