## Narrative

An employee was operating a roll form machine and attempted to remove glue from one of the rollers using a rag. The rollers caught the rag, causing the fingers on the employee's right hand to be pulled into the roller and crushed resulting in a partial amputation to the ring finger. The machine was locked out/tagged out at the time.
