## Narrative

An employee was working on a 60-kilowatt diesel generator in an outside workshop. He noticed some squeaking while checking the belts on the engine. As he tried to spray lubricant on the belt, the fan caught his right index finger, causing a large laceration. The finger was medically amputated above the second knuckle. The fan was not locked out/tagged out at the time of the incident.
