## Narrative

An employee was working on a hydraulic cylinder on a trackhoe. A strap securing a cylinder slipped and the cylinder struck and pinched the employee's left middle and index fingers which were inside the piston rod eye hole, causing a partial amputation of both fingertips at the nail plate.
