This administrative penalty order is issued to the Bethany Lutheran Home for the Aged and Gil Haugan Constructin Company for failure to obtain a stormwater permit prior to commencing construction activities, failure to develop a storm water pollution prevention plan, failure to install and/or maintain sediment and erosion controls, failure to implement permanent control measures and stabilization measures; and failure to conduct and/or maintain records of inspections.
