Narrative

While walking into the dining manager's office from the stove area, an employee slipped on the threshold of the office door and landed on his left elbow and onto his back. Preliminary diagnosis was concussion without loss of consciousness, acute headache, cervical sprain, thoracic spine sprain, and lumbar spine sprain.
