Narrative

At approximately 8:05 p.m., an employee suffered a possible amputation. The employee was sweeping mail at bin #1 and her finger was caught in the belt. The finger was then dragged between the belt and roller, resulting in a 2-cm avulsion to her middle finger on her right hand.
