Narrative

Employee sustained a foot injury on 1/14/15 when he stepped on a staple. Employee continued to report pain in the foot. Employee visited the emergency room on 1/24/15. Employee reported to work on 1/26/15 with continued pain in the foot and was sent to the doctor. The employee was subsequently hospitalized for treatment of cellulitis.
