Narrative

On or about January 27, 2015, at 3:45 p.m., an employee was hospitalized for surgeries related to the amputation of the two end phalanges of his index finger and the tip of his thumb on his right hand. He was injured while using a table saw to cut wood. The piece he was cutting kicked back, and the employee's hand fell into the saw blade. Using a table saw was a biweekly task of about an hour at a time.
