Narrative

Am employee working in the receiving area, sustained amputations of his right middle and ring fingertips on 2/18/15 at 7:30 PM. The employee's fingers were caught and amputated by receiving line gears while attempting to dislodge a cage. He was treated at Beebe Medical Center and hospitalized for 23 hours.
