Narrative

An employee was operating a rail saw and drill table when a hydraulic cylinder became detached from a push bar and the machine jammed. While attempting to unjam the machine, he placed his right foot between the end of the cylinder and the jammed push bar. The hydraulic ram then actuated and punctured his right foot.
