Narrative

An employee was setting a structure and was directing an excavator operator on where to dig a hole. He was standing between the bucket and a PCP (concrete) pipe. The operator could not see him behind the bucket. When the operator moved the bucket, the employee was pinned between the bucket and the pipe.
