Narrative

On or about 3/31/2015, at 9:40 a.m., an employee was picking up garbage. The employee was walking toward a garbage can when he slipped and fell on his back. Coworkers helped him up and kept working until the employee fell again and could not get back up on his feet. He was hospitalized.
