Narrative

On or about 16 April 2015 at approximately 0830 hrs an employee suffered an amputation of the distal phalanges located on the thumb of the left hand. The employee was retrieving keys from a safe when the door was accidently closed pinning her thumb between the door and the frame of the safe.
