On Saturday May 2, 2015, at approximately 9:00 a.m. an employee was lowering a 2000 lbs mold onto the assembly table in the assembly area with assitance from another employee. This was a second mold part that the employees were working on that morning. This mold was standing up on the assembly table approximately 3 ft. high. This mold was lowered with an overhead crane and attached straps onto the four 2x4 supports located on the assembly table. The straps were removed and the mold slipped off the supports subsequently falling off the table and striking the employee resulting in a severe left arm laceration (skin graft) and multiple fractures in the right arm, hand, and fingers.
