Narrative

On or about May 15, 2015, at approximately 8:00 a.m., an employee was trying to troubleshoot a hopper that was not filling properly. She took the cap off of the hopper and pulled back on an air cylinder. The air cylinder was on a timer; it actuated as she pulled on it and caught her left ring finger in the movement, amputating the fingertip.
