Narrative

A maintenance employee was trying to free a log jam on the DDM6 machine. The machine was locked and tagged out at the time. He was using a sledge hammer to break the log loose. The sledge hammer slipped, and the employee's left thumb became stuck between the hammer head and the blade on the machine.
