Narrative

On Friday 6/26/2015, at 9:00 a.m., a maintenance employee was driving, putting a brand new pin into the backhoe/excavator bucket with a slug hammer. A piece of metal from the new pin broke off, struck him and was imbedded in his shoulder. The injury required hospitalization.
