Narrative

A metal pipe line was plugged and the injured employee was attempting to remove the plug when the rotary valve positioned nearby was actuating and caught the air line the employee was holding pulling the gloves the employee was wearing into the actuating valve. The employee suffered a partial amputation (bone loss) of the right hand pointer finger up to the first joint.
