Narrative

On or about August 6, 2015 at approximately 8:16 am, a worker was cutting a piece of brick or cement when his saw got stuck. Employee tried to remove the saw by pulling, when he was struck by it on the face. Employee sustained cuts on the left side of his face near the mouth and jaw.
