Narrative

On or about August 13, 2015, an employee was painting an elevator door. She got the door key to open the door. As she pushed the heavy door, it opened, and she fell 20 to 25 feet to the bottom of the elevator shaft, suffering sprains and strains. No fall protection or lockout protection was used at the time.
