Narrative

Employees working on a roof took an extension ladder apart. The piece with the feet was being used on the roof to access another portion of the building while the other piece of the ladder was being used on the ground. The piece of the ladder being used on the ground was not tied off and fell when an employee used it. The employee broke his leg and was hospitalized.
