Narrative

On August 13, 2015, at about 5:30 p.m., an employee was operating a center rider (forklift) to pick up a pallet with his left hand outside of the forklift. He smashed his left-hand little finger against a bollard pole at the warehouse. His fingertip was surgically amputated.
