Narrative

An employee was connecting a concrete form that was 29 inches high x 30 feet long. The form was being stored on the ground horizontally. Before the employee could get the form connected to the overhead gantry crane, the form fell over and injured the employee's right leg, causing a distal fracture to the right femur.
