Narrative

An employee inserted an Allen key into the soft step adjuster of a lip groover to adjust the machine's tension. His left pinky finger was positioned near the machine shaft and shaft housing. The work cell was not de-energized at the time. As he was making the adjustment, the machine cycled, pulling the shaft into the housing and amputating approximately half of his left pinky finger.
