Narrative

An employee at the warehouse placed a 35-pound box of degreaser on the forks of a motorized stand-on powered industrial vehicle and hit the wrong button, causing the PIV to move backwards and pin the employee against the rack system. The employee suffered a crushing fracture to the left leg.
