An employee was operating a carton filler machine when the machine began to jam. While attempting to clear the jam, the employee's hand contacted the moving chain conveyor system. The employee's left-hand thumb fat pad, first index finger knuckle, and middle finger middle knuckle were amputated.injuring his head
