Narrative

An employee operating a lathe attempted to remove a shim located between the jaws of a chuck and pipe. While the employee was removing the shim, the kicker actuated, causing the pipe to move. The employee's finger became stuck between the pipe and the chuck jaws, amputating the right index fingertip.
