On September 17, 2015, at approximately 6:30pm, a worker was injured when the tip of his right index finger was pinched while changing the blade on a horizontal band saw. Lockout was used to de-energize the machine. As the employee manually turned the wheel to engage the blade, his finger was pinched between the wheel and the blade, resulting in amputation.
