Narrative

On 10/16/15 an employee removed the guarding from the chain on a fan fold machine while the machine was running. The employee was attempting to replace the chain after the chain had come off the machine. The employee got his fingers caught in the machine. On 10/19/15 the employee had surgery for a partial traumatic transphalangeal amputation of the left index and middle fingers.
