Narrative

An employee was attempting to align the hydraulic mast tilt cylinder on a forklift so he could insert the locking pin. His finger was in the pin hole while he instructed the forklift operator to lower the hydraulic mast so the pin hole would line up and the pin could be inserted. He suffered a partial amputation of his left index finger.
