Narrative

On 10/22/2015 at approximately 10:10 pm, a forklift operator stopped his forklift and placed it in park. The employee took a drink of water that caused him to cough and presumably choke. The coughing, caused the employee to lose consciousness and fall from the forklift suffering eight broken ribs and a lacerated spleen.
